Eastgate have just released a product based on years of research in the hypertext community into spatial hypertext/idea structuring tools:

This is Mac only, not open source. However, all files are in XML, so could be a cool visual interface you graft onto other systems, and it claims comprehensive web export.

Free demo available to download and run in Classic (OS X coming).
