As I was checking out http://www.sgmltools.org, I came across two
other highly relevant references.

The first, is to a document called "On the Road to XML: Remaking the
LDP as a Digital Library", which charts a course for moving the Linux
Documentation Project to SGML and XML.

The second is much more exciting, and is called the Casbah Project.
Run by the North Texas Linux Users Group, it is an Open Source
development effort towards exactly the kind of co-evolutionary
development system we've been talking about. In particular, check out
the Casbah Swiki, a collaborative website intended to develop a
knowledge base for the project!
