This is key. Right there, you have it.
Tom Gilb has a concept of "inspection", where you define a quality
measure and apply it to the documents coming IN to you process, as
well as those going out.
The final exam is one measure of quality. But perhaps as you say
a better measure is performance in the next course. Or maybe the
first week of every course should be devoted to a review of the
prerequisite material, followed by a brief exam that covers the
minimum required knowledge. Perhaps a failure to pass that
"input quality inspection" should send the student back for a
refresher, and reflect on the performance of the course they
came from.
